If you want to renew a Sports Betting, Master, Operator, or Internet Operator License, you will need to complete the Sports Betting Business License Renewal Application. To complete the electronic application, it must be downloaded first.  Then it may be filled out and saved on your device.

  3. Criminal History Dispositions (if applicable)
  4. Additional supporting documents, if required:
    1. IAGR Multi-Jurisdictional Personal History Form 
    2. Colorado Rider to IAGR Form
